    Parents or Guardian must accompany students to enroll and provide the following items at the time of registration: Be sure all items are secure at the time of enrolling.  We are unable to schedule a student until all items have been cleared.   

    • Parental identification (must match parent on birth certificate) - Photo ID required
    • Alabama blue immunization form - Out of state forms cannot be accepted.  Please call the  Coffee County Health Dept or Lyster Army Hospital for instructions on obtaining an original blue card.
    • Proof of residency in legal guardian's name - 2 current items are required. (1st Item:  Lease or Mortgage Statement / 2nd Item:  Utility bill, Driver's license, Tag Receipt are acceptable forms of residency verification)   If residency documentations are not in legal guardian's name, additional clearance must be obtained from the central office prior to registration.
    • Copy of birth certificate - for age and custody verification
    • Copy of social security card or SSN requested for transcript tracking purposes
    • Withdrawal form or most recent report card from former school to verify grade and course placement (cannot receive a schedule without this).  Please ask the previous school to provide at least a withdrawal form until official records can be requested.
    • Copy of IEP or 504  if applicable
    • Custody documentation if applicable (must be signed by a judge)
