The following items must be provided at the time of registration:

• Alabama blue immunization form
• Proof of residency (utility bill, lease agreement, etc)
• Copy of birth certificate
• Copy of social security card
• Withdrawal form from former school or most recent report card
• Copy of IEP if applicable
• Custody documentation if applicable
